If you would like to share your record, please inform a member of staff. For further information please click on the link below:
Friends and Family Test
If you have visited the surgery recently, then please use our survey listed on the right to indicate if you would recommend our service to your friends and family, alternatively ask at reception for a paper survey.
These results will be collated and published from February 2015 on a monthly basis.
Pharmacy First Scheme
Did you know that other health care providers in the local area may be able to help with your healthcare needs?
Simple winter ailments such as coughs, colds, diarrhoea, sore throats and headaches are now being dealt with at your local chemist. Under a new scheme, qualified staff at Boots and Lloyds are now able to prescribe some medications for minor illnesses. As well as winter ailments they can also treat conjunctivitis, impetigo, nappy rash, uncomplicated urinary infections and oral thrush.
If your health is usually good then this service may be of use to you if you become unwell. Ask at your chemist for details.
Patients over 75
You may be aware that from April all practices are required to provide all their patients aged 75 and over with a named GP who will have overall responsibility for the care and support provided by our surgery. This does not prevent you from seeing any GP of your choice.
If you have any questions, or wish to know who your named GP is, please contact the surgery.
Facilities for the Disabled
There is good access into the surgery for wheelchairs and the team are ready to assist patients as required.
Friends of Hartland Practice
We are very fortunate to have the above group of people chaired by Mr John Scott who arranges meetings and fund raising events for the Practice. If you would like to be put in touch with them, speak to the Practice Manager who will pass on your details.
Patient Participation Group
We have a virtual group who we email information to and ask for feedback from. If you would like to be part of this group to help develop the service we offer you please email D-CCG.Hartland@nhs.net
Please be aware that this email address is not constantly monitored and should not be used for medical queries.
